Add diced red onion; sauté until translucent (3-4 minutes). Add spicy Italian sausage, breaking it up as it cooks until browned (5-7 minutes). Stir in butternut squash and minced garlic; cook until squash softens (about 5 minutes). Fold in chopped Swiss chard, red chili flakes, salt, and pepper; cook until wilted (2-3 minutes). Boil water in another pot; cook rigatoni according to package instructions until al dente. Drain rigatoni (reserve some pasta water). Combine with the skillet mixture along with creme fraiche and Parmesan cheese; stir well. Adjust creaminess with reserved pasta water if needed. Serve hot, topped with extra Parmesan if desired.
